Complete and submit the following list of required forms. The required forms can be found on the bottom half of the main ADM employment page.
- Technology Acceptable Use Policy (read, review, and sign)
- Direct Deposit Form
- Federal W4
- Iowa W4
- I9 – two support ID documents required as listed
Please submit all forms to Rikki Kuhns, Administrative Assistant to the Business Manager or mail/drop them off to her attention at 215 North 11th Street, Adel, IA 50003.

After you have submitted your application and required forms, a background check request form will be emailed to you.
Please fill out the background check request form within 7 days of receiving it in order to initiate the background check process.

Directions: All required training courses listed below must be completed prior to employment start date.
Please coordinate a time with Rikki Kuhns to complete your required training courses at the district office or to ask any questions about the trainings.
List of Required Trainings:
- Bloodborne Pathogens Certificate (valid for one year from date taken)
- Right to Know – Hazardous Chemicals Certificate (no expiration date)
- Chapter 103 – Overview of Seclusion and Restraint Certificate (valid for three years from date taken)
- Mandatory Reporter – Child Abuse Certificate (valid for three years from date taken)
